Real Shit Nobody Tells You – Bowels, Babies and Biological Secrets

Buckle up for the real shit nobody tells you – bowels, babies and biological secrets or: why constipation isn’t just an icky inconvenience – why it’s a health issue that can affect your fertility, gut health, and overall well-being. As a fertility coach and midwife, I’ve seen how this often-overlooked issue can disrupt fertility, post-natal recovery and even lead to serious complications like diverticular disease.  Let’s talk about why constipation matters, how it connects to fertility for both men and women, and what you can do to truly get things moving – beyond the tired advice of “drink more water and eat bran.”

Why Constipation Deserves More Attention

The Germans have a wonderfully pragmatic approach to gut health—they treat their bowels with the same seriousness as their hearts or lungs. And they’re right! Your gut is central to your overall health. Chronic constipation doesn’t just make you feel sluggish; it can lead to inflammation, toxin build-up, and hormonal imbalances that directly impact fertility.

For women, the bowel plays a crucial role in eliminating excess estrogen. When waste lingers too long, estrogen can be reabsorbed into the bloodstream, worsening conditions like endometriosis, adenomyosis, or PCOS—all of which are linked to infertility. For men, constipation increases pressure on the prostate and raises oxidative stress levels in the body, which can damage sperm quality and motility.
And then there’s secondary infertility – which can be a hidden consequence of chronic constipation.
Straining weakens pelvic floor muscles. It can lead to adhesions or prolapse after childbirth  and especially c-section delivery, making conception more difficult than it should be the second time around. That’s why stool softeners are invaluable in your hospital bag for those first post-natal poops.  Too many women still face the horrific trauma of Manual Evacuation of Faeces ( exactly as bad as it sounds) due to lack of bowel care. . Followed by expert post-natal pelvic floor physio.

The Gut-Fertility Connection

Your gut is far more than just a digestion machine—it’s a command centre for your entire body. Giulia Enders’ brilliant and funny book ‘Gut: The Inside Story of Our Body’s Most Underrated Organ’ explains this beautifully. She describes how the gut is deeply connected to immunity, mental health, and even hormonal regulation. Enders highlights the importance of your microbiome – the trillions of bacteria in your digestive system—and how its balance affects everything from mood to metabolism.

What does this mean for constipation? It means that drinking gallons of water or eating mountains of bran isn’t the whole answer…

In fact, for some people, these approaches can make things worse by causing bloating or irritation if the underlying issue isn’t addressed. A healthy gut isn’t just about fibre; it’s about balance – of bacteria, hormones, and habits.

Men’s Fertility: Why Bowel Health Matters

Men often don’t realize how much their gut health impacts their fertility. Chronic constipation increases pressure on the prostate (which sits right next to the rectum), potentially leading to inflammation or enlargement that affects sperm production. Poor gut health also raises oxidative stress levels in the body—damaging sperm DNA and reducing motility.By improving bowel habits and supporting microbiome health with probiotics or fermented foods like kefir (a favorite in Giulia Enders’ book), men can take simple steps toward better reproductive health.

Why Stress Slows Everything Down

Your gut has its own nervous system (the enteric nervous system) that communicates directly with your brain via the gut-brain axis. Stress disrupts this connection, slowing down gut motility—the speed at which food moves through your digestive tract—and leading to constipation. This is why stressful periods like fertility treatments or post-natal recovery often trigger bowel issues.

Hypnotherapy offers a powerful way to restore balance by calming the nervous system and improving gut motility. Through guided imagery and relaxation techniques, hypnotherapy helps “retrain” your brain-gut connection. Imagine visualizing gentle waves clearing blockages in your digestive system or warmth spreading through your abdomen. At Manchester General Hospital these techniques have been shown to improve symptoms like bloating and irregular bowel movements by up to 70%. It’s a gentle yet effective way to address both physical and emotional triggers of constipation.

Practical Tips for Smooth Digestion

Here’s how to keep things flowing naturally:

  1. Balance Your Diet
    • Add smoothagefoods like pears, kiwis, spinach, oats, and flaxseeds—not just bran! Too much fibre without enough fluid can backfire.
    • Include fermented foods like kefir or sauerkraut for microbiome support.

  2. Hydrate Wisely
    • Sip fluids steadily throughout the day rather than chugging large amounts all at once.

  3. Move Your Body
    • Gentle exercise like walking stimulates bowel activity naturally.

  4. Relaxation Techniques
    • Yoga, meditation, or hypnotherapy can calm stress-induced gut sluggishness.

  5. Probiotic Support
    • A high-quality probiotic tailored to your needs can help restore bacterial balance.

  6. Routine Matters
    • Train your body by going at the same time each day- even if nothing happens at first! Use a potty stool for better evacuation.

  7. Know Your Poop: The Bristol Stool Chart

Your poop is one of the best indicators of your digestive health so it’s it’s time we all got over the ‘Ick factor’ talking about it!

Developed by doctors at the Bristol Royal Infirmary in 1997, the Bristol Stool Chart is a simple tool that helps you understand what’s going on inside your gut by categorizing stool into seven types:

    Type Description What It Means
Type 1 Hard lumps like nuts (difficult to pass) Severe constipation
Type 2 Sausage-shaped but lumpy Mild constipation
Type 3 Sausage-shaped with cracks on the surface Normal but leaning toward constipation
Type 4 Smooth and soft like a snake or sausage Ideal stool consistency
Type 5 Soft blobs with clear edges (easy to pass) Lacking fibre
Type 6 Fluffy pieces with ragged edges (mushy) Mild diarrh0ea
Type 7 Entirely liquid (no solid pieces) Severe diarrhoea

Types 3 and 4 are considered ideal—soft, easy to pass, and regular. Types 1 and 2 indicate constipation, while types 6 and 7 suggest diarrhea or rapid transit through the bowel. Understanding where you fall on this chart can help you identify patterns and make adjustments for better gut health.

Final Thoughts: It’s Time to Take Your Gut Seriously

Constipation isn’t just an inconvenience – it’s a useful signal that something deeper needs attention.

Whether it’s hormonal imbalances in women or prostate health in men, addressing bowel issues can have profound effects on fertility and overall well-being.
As Giulia Enders says in Gut, “The gut is not a dirty topic – it’s where health begins.”
So let’s stop whispering about constipation like it’s embarrassing! Your body deserves care- and so does your future family.
